Following the link takes the lead to a confirmation page that records the opt-out clearly, with a success page confirming they're out. No login, no preferences maze, no dark patterns.

Suppression does the rest

An opted-out address joins the suppression list alongside bounced and complained addresses, and every campaign checks that list before sending. Opt out once, and no campaign from your account will mail that address again.
